A technician photographs a defect and a work order is drafted on the spot: fault class proposed by a model, asset resolved from the register, priority and SLA clock set by a deterministic matrix, permits attached by rule. The model proposes and must clear a confidence threshold; below it, the photo goes to a person instead of a guess. A named human approves every dispatch, and every step lands in a hash-chained trail you can read at the bottom. Simulated captures, synthetic asset register, no real client data. Two of the six are chosen to fail well: one is rejected at the quality gate, and one is too ambiguous for the model, so it refuses and routes to a person. The refusal is the feature.
Priority is not the model's opinion. It is read off this matrix: the model's fault class carries a severity, the register carries the asset's criticality, and the cell where they meet fixes the priority, the response clock and the rectification clock. Change either input and the priority changes with it, explainably.
The register the pipeline resolves against: twelve assets on a synthetic three-storey estate. Criticality drives the SLA matrix; warranty status decides whether the order routes to the term contractor or the OEM.
